@thezimguy -  Great!  thanks for the response.  I will take a look at this and see if I can use it to develop one of my own.  Also, what's the
password for the DB?   When entering the program it asks me for one to connect to the SQL DB.

Yes, also @thezimguy - it appears the folder tree is incomplete.  Your forms make reference to a folder named 'icons' that contain images for one
of the forms and is not included with the zip file.

Hi Colleagues, you first have to install a MySQL database on your PC. You can use xampp to do that.
I added the zims.sql file in the zip which you will import into the database you create.

CONNECTION DETAILS (MySQL Connection)
The connection detail is what you setup in the phpMyAdmin
Example

Server(Host) : localhost
Port : 3306
User : root (default)
Password : (empty default)
Database : zims (the name of the database you created)

For the icons you can import any icon png of your own.
It has no effect on the system.
