Topic: Could somebody help me implementing this complex scheme?

Please see atachement. I would be interested if somebody could help me to implement the calculation process as seen under GI / Colon cancer or colonic polyps in this Word VBA document? I am an absolute beginner in terms of programming. Thank you in advance.

I looked at the page provided.  I don't see the actual source to the macros - therefore not  sure how to use the page or what the calculations would need to be implemented.   More info and/or details are needed.

Please find macro source code. I would like to ask for some example of some clickable form in visual database n which - based on the ticked parts of the form - the final report would be generated.
